Job Description:
Responsibilities
- Primary responsibility for patient/family caseload
- Report On-Call activities to clinical leadership
- Complete care coordination notes for visit notes and clinical
information
- Assist in delivering medications and supplies after business
hours
- Attend interdisciplinary team meetings for full team
discussion
- Admit eligible patients for hospice services
- Report patient status to clinical and hospice physician
- Develop patient care plan addressing identified problems
- Ensure use of 4Ms and provide Age-Friendly Care Qualifications
- Current RN license in state of practice or Nurse Licensure
Compact (NLC)
- Valid driver's license and automobile with insurance
- Two years recent nursing experience
- Ability to meet physical demands
- Ability to work independently and make sound nursing
judgments
- Knowledge of needs of terminally ill patients and their
caregivers
- Effective communication skills
